We have you fill this out when you come in for a "new patient appointment" or if your last appointment with one of our doctors was more than a year ago. You may find it helpful to review the information we ask for, or fill this out in advance and bring it with you to your appointment.

If we have your records, we will ask you to sign and date a consent form for us. Our fax number for release of patient records is 970-493-0521.
We have two forms for patients to use -- one if they want to receive their records themselves, and a second if they want OCR to send their records to another doctor, clinic, etc.

We keep patients' charts for about 8 years. If your surgery or office appointments are older than that, it is possible we no longer have your records.
If you had surgery at an area hospital, the hospital may have your old chart. Check with their medical records department. If your file is still available, you will need to sign a release form for the hospital.
Total Joint Surgery Preparation Classes
OCR patients who are planning to have a total joint replacement, or who want more information about these operations, can attend a free class to prepare for the operation or learn more about it.
Poudre Valley Hospital offers free classes on this schedule: Total Hip or Knee Replacement Class This class takes place every Monday from 2-4 p.m. at the Westbridge Medical Suites, 1107 S. Lemay Ave., across the street from the hospital. The meeting location is in the Westbridge Conference Center on the 1st floor. No registration required. Note that the class is not held on major holidays that fall on Monday.

McKee Medical Center provides this class schedule: Total Joint Education McKee's class meets every Thursday from 3-4 p.m. in the McKee Conference and Wellness Center. Your spouse, other family member, or caregiver is welcome to join you. No registration required. If you have a question, contact McKee at 970-635-4172. Medical Center of the Rockies offers this class: Joint Replacement Surgery Pre-Operative Class MCR's class is held twice a month, from 6-8 p.m. on the second Tuesday and from 2-4 p.m. on the fourth Tuesday of every month. The location is the Mount Meeker Conference Room on the lower level of the hospital. For more information and to pre-register, call 970-624-4358.
