Here is a link to our Patient Health History form. We have you fill this out when you come in for a "new patient appointment" or if your last appointment with one of our doctors was more than a year ago. You may find it helpful to review the information we ask for, or fill this out in advance and bring it with you to your appointment.

Need Your Old Medical Records?

To learn whether we have your chart, contact us at 970-419-7015, or call 1-800-722-7441 toll-free and ask for our medical records department. Or send us an email via This e-mail address is being protected from spambots, you need JavaScript enabled to view it . If we have your records, we will ask you to sign and date a release of records form for us. Our fax number for release of patient records is 970-493-0521.

We keep patients' charts for about 8 years. If your surgery or office appointments are older than that, it is possible we no longer have your records.

If you had surgery at an area hospital, the hospital may have your old file. Check with their medical records department. If your file is still available, you will need to sign a release form for the hospital.

Total Joint Surgery Preparation Classes

OCR patients who are planning to have spinal surgery or a total joint replacement, or who want more information about these operations, can take advantage of free classes to prepare for the operation or learn more about it.

Poudre Valley Hospital offers free classes on this schedule:
Total Hip or Knee Replacement Class -- this class takes place every Monday from 2-4 p.m. at the Family Medicine Center building, 1025 Pennock Place (2 blocks north of PVH). The meeting room is Suite 118 (lower level). No registration required.

McKee Medical Center provides this class schedule:
Total Joint Education -- this class meets every Thursday from 3-4 p.m. in the McKee Conference and Wellness Center. Your spouse, other family member, or caregiver is welcome to join you. Medical Center of the Rockies offers these classes:
Joint Replacement Surgery Pre-Operative Class -- this class is held twice a month, from 6-8 p.m. on the second Tuesday and from 2-4 p.m. on the fourth Tuesday of every month.
