OCR MyChart: Existing Patient Online Scheduling
OCR has transitioned to a new electronic medical record system called Epic, as of February 2, 2026. With Epic’s mobile platform, OCR MyChart, you’ll enjoy an improved patient experience with better coordination across your healthcare team, no matter where you are.
Existing patients of OCR can schedule an appointment online using OCR MyChart. If you a new patient and want to schedule an appointment at Longmont, Lafayette, or Westminster, you can schedule online with this link. If you a new patient and want to schedule an appointment at Fort Collins, Loveland or Greeley, you will need to request an appointment using the form below and a member of our scheduling team will reach out to confirm the appointment details. If you already have a MyChart account through UC Health, you can login using your same credentials.
